Before the advent of fat grafting, the technique employed the use of solid silicone implants, which is still used by some today. However, the technique is more difficult, more painful postoperatively and fraught with more complications, and it needs longer post-op recuperation. The results are also never quite natural because of the presence of a foreign body. The use of fat grafting has revolutionized buttock augmentation. It is simpler to perform, with a usual operative time of one and a half hours. It has minimal pain and discomfort postoperatively and requires minimal recuperation.

An additional benefit is that in removing fat from the donor sites to be used for the grafting, such as the abdomen, the patient also benefits from modified liposuction to the donor site.

In order to use a fat graft for the buttock enlargement, it is necessary that you have sufficient fat quantities in the donor regions, such as the abdomen. If you do not have enough fat, you may need to seek augmentation with buttock implants.

After fat is harvested from donor sites, it is processed and then placed in the buttocks. It is carefully layered throughout the buttocks in small quantities to improve the likelihood that the fat will survive. However, in about 30% to 35% of patients, a significant portion of the fat is absorbed, leading to poor results or often not enough correction or else there is dimpling or unevenness. In these cases, a second treatment is necessary to get the desired result.
